 I hereby request that IN Home Inspection, Inc. conduct a wood destroying pest inspection and report. The INSPECTOR will conduct a wood destroying insect inspection and submit a Form NPMA-33 (National Pest Management Association) Wood Destroying Insect Inspection Report. This is not an inspection for health related issues. Form NPMA-33 is for insect reporting only; wood decay fungi, or conditions conducive to infestation may be noted in Section V of the report or on a separate wood destroying organism attachment to the wood destroying insect infestation report (Form NPMA-33)
